The total market saw significant expansion over the last decade, with direct earned premiums reaching $1.7 trillion in 2024. This concentration of market power highlights the scale of national brands in managing care and prescription costs for over 270 million people.

UnitedHealth Group remains a dominant force, utilizing a network of 1.5 million healthcare specialists and 6,200 hospitals to serve over 49 million members. The company operates through two primary units, UnitedHealthcare for insurance services and Optum for health services, providing benefits nationwide.

CVS Health has also emerged as a major player by integrating Aetna’s insurance business with its extensive pharmacy chain and pharmacy benefits management services. This diversification allows the company to write health policies across various segments while leveraging its retail and PBM infrastructure.
